Parking & Office Location Information:
We have two office suites at our Fort Worth hospital location.
Parking:
Please park on the north side of the hospital in the Plaza Offices parking lot, next to the Breast Center. If you are at the ER entrance (south side), you are on the opposite side of the hospital.
Our Office Locations
- Suite 140: Plaza Offices, First Floor – Main Office
- Suite 265: Plaza Offices, Second Floor
How to Find Suite 265
- Enter through the glass doors from the Plaza Offices parking lot. This is the same entrance you use to go to Suite 140.
- The elevators are immediately to the left as you walk in.
- Take the elevator to the second floor.
- Turn right to find Suite 265.
Check-In Instructions
Unless you are told otherwise by our staff, please check in at our main front desk in Suite 140.

Directions for Parking:
Our office is located off Hill Blvd. behind the Walgreens. As you approach our office building, you will veer to the left and our entrance is on the left side of the building. Please park in any available parking spot.
